a) are the voices on the top, or the instruments?
b) do the lyrics go under the top voice line or the bottom? (of three voices)

It depends. Is it a true, written out guitar part, or just chords?

I would put the voices on top. I like to put the lyric line under each voice part. I find it less confusing, although, as a bass, I'm quite used to having them above. But if I was singing the top line, I wouldn't want my lyrics two or three staves away.

With multiple verses, I would write it out. I hate having to backtrack and keep an eye on which verse I'm in.

Accompaniment should go below the voices.

I'm with Mike on this, though depending on the guitar part, I might put the guitar above and have the voices in between guitar and piano. So if the guitar were just accompaniment, I'd do as Mike says, but if it were a kind of obbligato part with interesting material of its own, then it might be better positioned above the voices.
